Signing Up For Health Plans

Want to sign up for Community Health Choice?

You’ve come to the right place. This is our enrollment website to sign up for Health Insurance Marketplace coverage.

Right now, Health Insurance Marketplace rules permit us to only accept enrollments from people who qualify for a Special Enrollment Period.

Certain changes in your life situation can make you eligible for a Special Enrollment Period. You might qualify for a Special Enrollment Period within 60 days of losing job-based coverage, having a baby, getting married, or certain other life changes. If you think you may qualify, answer a few quick questions to find out. Or, call us at 713-295-6704 (toll-free: 1-855-315-5386), Monday – Friday, 8:00 a.m. – 5:00 p.m.

More Information

Qualifying Life Event

A change in your life status makes you eligible to enroll in health care coverage through a Special Enrollment Period. Examples of qualifying life events are moving to a new state, certain changes in your income, and changes in your family size (for example, if you marry, divorce, or have a baby).

Special Enrollment Period

Any time outside of the Open Enrollment period, during which you and your family have the right to sign up for health care coverage, is considered Special Enrollment. In the Marketplace, you generally qualify for a Special Enrollment Period of 60 days following certain life events that involve a change in family status (i.e., marriage or birth of a child) or loss of other health coverage. If you do not qualify for the Special Enrollment Period, you cannot purchase insurance through the Marketplace until the next Open Enrollment period.
